| IS BOTOX® FOR YOU? |
| Botox® is used to disable the muscle which allows the wrinkles to gradually disappear in areas such as the brow for frown lines, around the eye area for crow's feet, and the forehead area. However, Botox® should not be used during pregnancy, ifyou are nursing, or if you are taking certain medications. Please consult with Dr. Choi to determine the best course of your Botox® therapy. |
| UNDERSTANDING BOTOX® INJECTIONS |
| Botox®is recognized as a safe and effective treatment for problems such as uncontrollable blinking, twitching, and other spasms as well as with wrinkling. During the therapy, Dr. Choi uses a very fine needle to inject Botox into the specific muscle, so there will be minor and temporary discomfort. Usually you will see the effects of BOTOX® within three to five days. A single treatment of BOTOX® will normally be sustained for approximately three to five months. You will notice a gradual fading of its effects. Normally, BOTOX® treatment is required three or four times a year. |
| AFTER THE INJECTION |
| You may experience a little or no redness/swelling for about three to five hours. There are very few if any side effects with BOTOX®. Side effects are usually mild and temporary: Temporary weakness of muscles or discomfort at the injection site, bruising, irritation of the injected area, drooping of the eyelid, and so on. |

| UNDERSTANDING THE BRAVA® SYSTEM |
| To
use the Brava® Breast Enhancement and Shaping System, you will
meet with Dr. Andrew K. Choi for an initial consultation. This initial visit
allows Dr. Choi to answer any questions about the use and operation of the
System. At this time Dr. Choi will also evaluate your medical history to
determine if you are a candidate for the System. Dr. Choi then measures
your chest and breast size to order a custom-fitted system.
Within a few days, the Brava System is delivered to our office. The complete system includes two pairs of domes, a computerized controller box, a sports bra and tote bag, and instructional material. To monitor progress, the System offers an on-line component that enablesa both Dr. Choi and you to access usage patterns and information. Access to your information is achieved by logging on to the "Member information" section of the web site, www.mybrava.com, and entering a secure PIN access code. Most women wear the system at home and sleep with it on to comply with the ten-hour protocol. Although it is comfortable to war, some women need a few days to adjust their sleeping position if they do not sleep on their back. The most common reaction can be a mild skin rash that may occur from contact with the surface of the system's domes. |
| AFTER THE BRAVA® SYSTEM |
| In
clinical trials, women who used the Brava System for the prescribed ten
week, ten hour a day period, experienced a gradual and lasting increase
in breast tissue of about 100cc- about a 50% increase in volume or approximately
one cup size - as measured 15 months after they had stopped wearing the
system. In addition to increasing breast volume, all of the women reported
a noticeable lift of their breasts.
All participants also showed a significant increase in self-esteem following the use of the Brava System, based on both the Tennessee self-Concept Scare and an administered self-satisfaction survey. Women also reported that the Brava System is comfortable and easy to wear. |
| IS COLLAGEN® FOR YOU? |
| COLLAGEN® implants are made from cow skin that has been highly purified, and it helps as the supportive structure for your skin once injected. There are indications for treatment with COLLAGEN® implants which are frown lines, acne scars, cheek depression, vertical lip lines, marionette lines, worry lines, crow's feet, deep smile lines, and facial scars. However, COLLAGEN® should not be used if you are allergic to COLLAGEN® implant. |
| UNDERSTANDING COLLAGEN® THERAPY |
| There are three steps to get started with COLLAGEN® treatment: a skin test, the treatment series, and periodic touch-ups. To determine if you are eligible for treatment Dr. Choi will inject a small amount of collagen into your forearm and forehead, just below the skin's surface. You and Dr. Choi will be watching the test site closely for six weeks for any signs of reactions such as redness or swelling. You may find the injections are somewhat uncomfortable, particularly around the nose or lips. However COLLAGEN® implants contain a small amount of lidocaine that helps numb the area temporarily, and most patients report that the injections are relatively painless. Touch-up injections are usually needed to maintain maximum correction. Most patients get touch-up injections within three to six months of the original treatment series. |
| AFTER THE INJECTION |
| Most patients feel comfortable in resuming their normal activities following treatment. Temporary puffiness of the treatment areas could be expected. You may also notice temporary blushing, slight bruising, and tenderness around the treated sites. |

| AFTER THE INJECTION |
| You can expect to have some swelling and possible bruising after treatment with Cymetra™, and this is perfectly normal. Apply ice immediately and keep the treated area cold. Most swelling will subside in four to six hours. The majority of patients who receive Cymetra™ in the afternoon are comfortable returning to work or other activities the next morning. The duration of results will vary depending upon the patient and the location of the transplantation. |

| IS KELOID SCARS FOR YOU? |

Compared to laser treatments, Radiogrequency treatment is simpler and painless and gives less healing time. One or two times of treatment will clear your skin without scarring. |

| Avoiding sun exposure is the most important step to the best result. Dr. Choi will tell you not to take a shower for 7 days after each treatment and to apply a special ointment thinly 6 times a day. The purple blemish will fade in 7-10 days. From the 8th day, you must start applying a sun-block cream for at least 1 month. |

| Dr. Choi will advise you to avoid taking a shower or washing the treated area for the first five days after treatment. Until you notice your skin is back to normal, use a sunblock lotion and cover the treated area. In about 2 to 3 months, your skin color will be back to normal and you may resume with your normal activities. |
